The Case Manager is responsible for coordinating care and ensuring that clients receive the appropriate services and support needed to meet their health and wellness goals. Key Responsibilities:
Conduct initial assessments and evaluations to determine client needs and eligibility for services.

Coordinate services and resources, ensuring accessibility and continuity of care.
Maintain accurate and up-to-date client records in compliance with confidentiality and documentation standards.

Qualifications:

Minimum Requirements:
Bachelor’s degree in social work, nursing, psychology, or a related field.
Minimum of 2 years of experience in case management or a related field.
