We are seeking an experienced Clinical Pharmacist to join our team at Grove Medical Centre. The successful candidate will work as part of our multi-disciplinary team in a patient-facing role, taking responsibility for areas of chronic disease management and undertaking clinical medication reviews.
You will provide primary support to general practice staff regarding prescription and medication queries, help manage the repeat prescriptions system and handle acute prescription requests.
A key aspect of the role involves medicines reconciliation on transfer of care and implementing systems for safer prescribing. You will work closely with GPs to provide medicines optimisation, quality improvement, CQC compliance and manage aspects of the Quality and Outcomes Framework and enhanced services.
The role requires integration with community and hospital pharmacy teams to improve patient outcomes and manage workload efficiently. You will be pivotal in enhancing the quality of care and operational efficiencies within our practice. While on-site work is preferred, we offer the possibility of hybrid working arrangements for the right candidate.
Master's degree in Pharmacy (MPharm) or equivalent qualification
Current registration with the General Pharmaceutical Council (GPhC)
Completion of CPPE (PCPEP training pathway) or be in progress towards completion
Minimum of one year's experience working as a Pharmacist in NHS General Practice or a Primary Care Network
Proficiency in using EMIS Web clinical system
Ability to prioritise workload and manage competing demands
Excellent interpersonal, communication, and negotiation skills
Ability to work independently and as part of a multi-disciplinary team
Strong analytical skills with the ability to interpret complex medical information
Experience in efficiently and safely completing medication reviews
Familiarity with NHS structures, policies, and guidelines related to primary care
Excellent organisational and time management skills
Commitment to continuous professional development
Understanding of information governance and patient confidentiality
