Home Housing Specialist

Overview:

Diversion Specialist primary responsibility is to divert eligible single women and families from accessing emergency shelter beds. This position requires the assessment of those seeking shelter for diversion eligibility, the identification of immediate alternate housing arrangements, and the connection of eligible women and families to appropriate services and financial assistance. Required to work flexible hours, including evenings, week-ends and provide back-up for shelter on-call rotation. Performs other duties as assigned.

Responsibilities:
  • Engage clients in problem solving dialogue to assess for additional resources client may be able to access within their own network.
  • Connect clients with appropriate partner agencies through referrals and follow-up to ensure connections are made
  • Tracks outcomes related to diversion activities, and assist, as needed, with the prevention of housing disruptions for past clients of the House of Mercy.
  • Spend considerable time with clients in immediate need and must be able to not only assess the situation, but provide comfort, brainstorm creative alternative solutions, and help meet clients’ immediate, basic needs.
  • Provide mediation assistance to encourage family reunification, conflict resolution and landlord, and other party reconciliation
  • Enter, update and maintain data in the HMIS “Homeless Management Information System
  • Conduct immediate and longer-term follow-up to ascertain the effectiveness of diversion activities.
  • Work within the community will also be required.
Education and Experience: High-school graduate, some college preferred.
1-3 years prior experience assisting the homeless and/or formerly homeless in a volunteer or paid capacity in a community or shelter setting. Prior lived experience (having experienced homelessness) and familiarity with assessment tools and the HMIS and/or equivalent automated data tracking systems and understand of trauma informed care.
Certification/Licensure:
For those driving Mercyhealth vehicles:
Maintain a valid driver’s license.
Have and maintain an acceptable driving record per Mercyhealth’s HR.100 Fleet and Driver Safety policy.

For those driving their personal vehicles:
Maintain a valid driver’s license and reliable transportation with proof of vehicle insurance when requested.
Have and maintain an acceptable driving record per Mercyhealth’s HR.100 Fleet and Driver Safety policy.

Special Physicial Demands:
The Special Physical Demands are considered Essential Job Function of the position with or without accommodations.
Lifting of boxes of donations and materials for community outreach events. Good manual dexterity to operate keyboard and telephone.
