Care Manager Registered Nurse
Hopscotch Primary Care is looking for a Care Manager Registered Nurse to join their team in Shelby, NC. They aim to provide high-quality primary care tailored to meet the needs of patients in rural America.
Skills
- Associate’s degree in Nursing
- Valid, active RN license
- 2+ years of clinical work experience
- 1+ year of utilization review or case management
- Experience with electronic medical record systems
- Knowledge of healthcare system and regulatory requirements
Responsibilities
- Manage end-to-end case management services for a defined panel of patients
- Build and maintain relationships across care team, patients, and providers
- Coordinate care, discharge planning, and post-discharge follow-up
- Advocate for patients and provide family education and support
- Support best practices in care management and clinical excellence
Education
- Associate’s degree in Nursing (required)
- Bachelor of Science Degree in Nursing or related field (preferred)
