Home Health Intake Sales Coordinator
Location: Golden Valley, MN
Full-Time and occasional weekend
Salary: $26 to $30 per hour
Home Health Care Inc is seeking an intake sales coordinator to join our team here in Golden Valley! As an intake sales coordinator, you will be speaking with and collecting information from clients and referral sources. The intake sales coordinator should also have a solid understanding of home health services and knowledge of respective clerical functions. This role would include developing and foster strong business relationships for continued growth and focusing on account management with our existing referral sources and supporting our community sales team

Intake Sales Coordinator Qualifications:
- Experience in a healthcare office, ideally home health setting
- Knowledge of medical/healthcare and insurance terminology is highly preferred
- Experience with home care admissions/intake is ideal
- Demonstrated effective verbal and written communication skills
- Comfortable navigating and utilizing electronic medical records and computer programs
Intake Sales Coordinator Responsibilities:
- Review all incoming referrals by conducting an initial screening to ensure they meet the defined admission criteria
- Contact referring locations to gather client medical information and necessary documentation
- Conduct screening and informational interviews with patients and their families as part of the admission process
- Enter client-specific information and communication notes into the electronic medical record system
- Track clients throughout their stay under our care, ensuring their satisfaction, and follow-up as needed
- Maintain a positive presence inside our referral sources' locations
- Consult with families and clients about our services
- Work collaboratively with our clinical staff to assure consistent message to the community and our clients
