Transition Assistant (Part-Time)

Our mission at Lifelong Access is to connect individuals with developmental disabilities to their community by helping them break down barriers through a lifetime of meaningful supports. From birth to end of life, Lifelong Access provides a continuum of services and supports for families, children, teens, and adults including residential housing, vocational development, supported employment, high school transition, behavioral health counseling and services, and a wide variety of pediatric therapy supports. Lifelong Access is seeking a Transition Assistant to support the High School Transition and Supported Employment Programs in a variety of opportunities for students to learn new skills and plan for a smooth transition from high school to adulthood. The Transition Assistant will assist job seekers with developmental disabilities attain and maintain a career path of their choosing. 

Pay:                $17.75/hour
Job Type:       Part-Time (up to15 hours per week) 


Essential Duties and Responsibilities 
  • Provide direct support in the areas of programming, mini discovery, and IEP meeting attendance, on-the-job coaching, and job development activities. 
  • Present information through community presentations/networking events. 
  • Collaborate with the Transition Coordinator to plan programming. 
  • Assist with transportation of program participants when necessary, utilizing company vehicles.
  • Order and organize supplies for Transition programming. 
  • Assist the Transition and Supported Employment teams with supports when needed. 
Requirements:
  • Minimum of high school diploma or GED.
  • At least one year of experience supporting people with disabilities. 
  • Familiarity with Microsoft Office applications (Word, Excel and Outlook) is required.
  • Ability to work weekdays and early weeknight availability.
  • Willing to travel to rural communities in McLean County.
  • Must be at least 21 years of age.
  • Valid Illinois driver’s license with safe driving record.
  • Current proof of automobile insurance.
  • Reliable transportation.
Working Conditions  
While performing the duties of this job, the employee is occasionally exposed to outside weather conditions.  The noise level in the work environment is usually moderate.

While performing the duties of this job, the employee is frequently required to sit at a desk and work on a computer for prolonged periods.  The employee is frequently expected to talk, hear, and possess clear close and distance vision; and use hands, fingers to reach or feel.  The employee must occasionally lift and/or move up to 15 pounds. 

While performing the duties of this job the employee is regularly required to talk or hear.  The employee will also frequently need to converse with adults – some who might be difficult to understand or use nonverbal forms of communications. The employee is frequently required to stand, sit; stoop; bend, push, pull, reach, and walk for periods of time.  The employee will frequently use hands to fingers, handle, or feel; and reach with hands and arms.  The employee must occasionally lift and/or move up to 15 pounds.
