LMSW Social Worker for Houton, TX

We are seeking a TX LMSW to work closely with our customers on a daily basis. Complete initial and ongoing discharge planning assessments, complete final discharge disposition and other regulatory required documentation in the Cerner ACM program. Two (2) years of nursing or social work experience acute hospital-based preferred, or two (2) years of experience comparable clinical setting.

Job Type: Full-time

Pay: $61,630.00 - $91,250.00 per year
