LSSI is hiring a full-time Families Together Case Coordinator, Monday through Friday from 9:00 am to 5:00 pm. As part of the Families Together Team, provides direction and case coordination with a whole family perspective to build the family’s ecosystem with sustainable results. Monitors safety, well-being and progress, and brings in additional resources as indicated within the context of the evidence-based models and using a trauma-informed approach.

Essential Functions:
- Coordinates the development of an individualized service plan for families in the Families Together Foster Care Program, from a whole family perspective, building the family’s ecosystem and concurrently planning permanency.
- Coordinates the implementation of service plan components related to the family, foster family and children/youth and within the team context, including facilitating regularly and ongoing child and family team meetings.
- Completes all tasks within the context of the evidenced based model, ensuring the models are implemented with fidelity. Interventions will apply principles of social learning theory, developmental psychology and trauma-informed care.
- Co-lead KEEP groups for foster parents or GenPMTO groups for parents.
- Completes other case coordination duties, including court testimony, advocacy, resource referrals and documentation as required by LSSI and DCFS.
- Participates in supervisory process for guidance, direction, support and approval of decision making, assessment, service planning and intervention. Presents cases for staffing or participates in staffings as needed, including weekly team meetings.
- Participates in the on-call/crisis intervention response for the TF team.
Position Qualifications:
In addition to a real passion for working with at-risk children and families, this opportunity requires:
- Requires a minimum of a Bachelor’s Degree in Social Work or a related Human Services field.
- A related Master’s degree is preferred.
- Requires a minimum of two (2) years of related experience.
- Child Welfare Employee License (CWEL) or ability to obtain.
- Training in KEEP and/or GenerationsPMTO or availability to participate in training, coaching and fidelity to certification once hired.
- Demonstrated ability to motivate families, youth and foster parents to participate in detailed interventions models.
- Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software.
- Valid Illinois driver’s license in good standing, proof of automobile liability insurance, bodily injury and property damage. Access to reliable transportation.
