Manager of Case Management - Houton, TX

Responsibilities:
- Conduct patient assessments to determine their needs and develop individualized care plans
- Provide mental health counseling and behavioral therapy to patients
- Collaborate with healthcare professionals to coordinate and manage patient care
- Monitor patient progress and adjust care plans as necessary
- Ensure compliance with patient care standards and regulations, including Medicare guidelines
- Maintain accurate and up-to-date patient records

Pay: $110,000.00 - $150,000.00 per year
