Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.The Referral Coordinator will assist Care Managers with referrals to Post Acute Services to increase efficiency in transition to their next level of care.Qualifications:EducationHigh School Diploma, requiredBachelor of Social Work, requiredExperienceWork experience in a physician's office or medical institution with exposure to managed care and primary care rules andpracticesOtherSkill in exercising initiative, judgement, problem-solving, and decision-making.Skill in developing and maintaining effective relationships with staff, doctors, nurses, patients, and managed care representatives. Skill in organizing work to achieve clinic goals and objectives. Ability to communicate clearly. Knowledge of governmental regulations and reimbursement requirements.
