Registered Nurse (RN) Case Manager, Float
At Our Hospice, we provide compassionate care to patients facing the end of life. We are looking for a dedicated Registered Nurse Case Manager (RNCM) to join our team.
Skills
- Registered Nurse License in Indiana
- Experience in health care
- Ability to provide patient care in diverse settings
- Strong communication and coordination skills
Responsibilities
- Make routine nursing visits to patients in various care settings.
- Assess patient needs and provide comprehensive care plans.
- Coordinate with interdisciplinary team for quality care delivery.
- Educate patients and families on care techniques and benefits of hospice.
- Maintain accurate records and documentation.
Education
- Associate or Bachelor Degree in Nursing (ASN/BSN)
