Quartet Health is looking for a Social Worker passionate about providing holistic care to those with serious mental illness in the Cherry Hill or Philadelphia area.

Excellent written and verbal communication
Active Licensed Clinical Social Worker in New Jersey
Ability to develop and implement care plans
Strong teamwork and patient-focus
Experience in caring for patients with serious mental illness and substance use disorders

Responsibilities

Performing assessments to understand non-clinical care gaps
Facilitating connections to community-based services & supports
Advocating for patients by helping complete applications and forms
Ensuring patient engagement with identified resources
Participating in care planning meetings

Education

Bachelor’s or Master’s Degree in Social Work
