Social Worker - New Jersey Preferred
Quartet Health is looking for a Social Worker passionate about providing holistic care to those with serious mental illness in the Cherry Hill or Philadelphia area.
Skills
- Excellent written and verbal communication
- Active Licensed Clinical Social Worker in New Jersey
- Ability to develop and implement care plans
- Strong teamwork and patient-focus
- Experience in caring for patients with serious mental illness and substance use disorders
Responsibilities
- Performing assessments to understand non-clinical care gaps
- Facilitating connections to community-based services & supports
- Advocating for patients by helping complete applications and forms
- Ensuring patient engagement with identified resources
- Participating in care planning meetings
Education
- Bachelor’s or Master’s Degree in Social Work
