Job Summary
Based on direction from the Panel Manager Supervisor and Associate Medical Director for Quality, provide telephonic outreach to patients to support UMMG quality, utilization, and operational goals.

Responsibilities*
PRIMARY RESPONSIBILITIES – note that these responsibilities are shared across all Panel Managers and each individual Panel Manager may not be responsible for all responsibilities at a given time.
Provide key performance metric reporting to clinics on a regular cadence. Support transitions of care efforts through scheduling patients with the appropriate provider within the appropriate amount of time.
Support financial sustainability of Care Connect and UMMG through supporting the transitions of care, Behavioral Health Collaborative Care, and Care Coordination billing processes.
Provide operational support to Care Connect team members, to include special project outreach and potentially scheduling support, which can include referral work queue management.
Other ad hoc responsibilities as assigned by UMMG.
Calling patients on various lists / reports to bring them back into the clinic either with Care Management or PCP.
Billing for any billable services – new BCBSM guidelines allow for MAs to bill for the 98968 (5-10 minute phone call), 99487, and 99489. MAs would selectively provide outreach to and bill the BCBSM Commercial plans.
Additional responsibilities / basic business functions:
Provide quality service based on customer expectations and needs.

Practice effective time management.
Participate in interdisciplinary meetings, task forces, and projects as appropriate.
Collect outcome data to help improve PM practice and demonstrate PM outcomes.
Required Qualifications*
High School diploma or equivalent.
Graduation from a Medical Assistant program or an equivalent combination of education and 3-4 years of experience.
Excellent attendance record and punctuality.
Must adhere to a high standard of personal and professional conduct.
Demonstrate excellent written and oral communication skills.
Must demonstrate the ability to interact with a diverse population.
Must be an active team player with the ability to work independently.
Possess strong organization and prioritization skills.
Adhere to high standards of teamwork by demonstrating adaptability, flexibility, and consistently sharing information and resources with others.
Demonstrates the ability to multi-task in a fast paced environment.
Proficient in Microsoft Word and Excel.
2-4 years? experience in a customer service environment.
Desired Qualifications*
Associates degree in the healthcare industry.
Experience working in a complex health care setting, scheduling patient appointments.
Basic knowledge of medical terminology.
3-5 years’ experience in a Customer Service environment.
Familiarity with MiChart.
Knowledge of University policies and procedures.
Work Schedule
Days: Monday – Friday
Hours: Varied shifts between 8am-5pm

Work Locations
Michigan Medicines North Campus Administrative Complex in Ann Arbor, MI. This position may be eligible to work from home once training is complete and performance metrics are met and able to be maintained).
