As a Private Duty Nurse at Phoenix Home Care & Hospice, your responsibilities include assisting in developing, reviewing, and revising the care plan, providing nursing care according to the physician’s treatment plan, observing and recording client reactions to treatment, and educating both clients and families about the care plans. You’ll also be documenting care services through electronic charting, and participating in team meetings and quality assessments. This role is essential in ensuring our clients receive the highest standards of personalized care.

To qualify for the Private Duty Nursing position at Phoenix Home Care & Hospice, you must hold an active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
