Position Overview:

The purpose of this position is to perform the primary functions of a physical therapist in assessing, planning, directing, and evaluating patient care in an acute care setting.

Essential Job Functions:
  • Addresses patient needs and provides quality care by evaluating functional performance and determining physical therapy treatment plans
  • Helps patients accomplish their treatment plan by instructing, encouraging, and assisting in performing physical activities, exercise, ambulatory functional activities, daily-living activities and fitting/use of assistive devices
  • Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending discharge disposition and assistive equipment
  • Document the evaluation, plan of care, treatment interventions, response and progress in patient’s medical record
Other Job Functions:
  • Attend staff meetings as required
  • Perform additional duties as assigned
Basic Qualifications:
  • Licensed Physical Therapist, Master’s Degree or higher
  • Current State licensure in good standing
  • Current BLS certification through the American Heart Association (AHA), required
  • At least one (1) year practicing as a licensed clinician in the acute care environment, preferred
  • Ability to work effectively with multi-disciplinary team to promote patient-centered and individualized discharge planning
  • Demonstrate effective quality focus, bedside manner, clinical skills, persistence, energy level and legal compliance
  • Position requires fluency in English; written and verbal communication
