Tele-Health Primary Care Provider (Remote)

Pine Park Health is seeking a telehealth Primary Care Provider to deliver care to a geriatric population. The ideal candidate will work remotely and collaborate closely with the primary care provider team to provide high-quality patient care.

Skills

Board-eligible or Board-Certified Nurse Practitioner with geriatrics experience
Experience in telehealth, urgent care, or ER is preferred
Strong written and verbal communication skills
Ability to work in a fast-paced environment and collaborate effectively

Responsibilities

Provide remote telehealth primary care to geriatric patients in senior living communities
Respond rapidly to urgent care needs to reduce hospitalizations
Assess clinical requests, triage appropriately, and provide timely resolutions
Support a set number of patients per day, offering urgent care via telehealth
Collaborate with primary care providers and support team communications
Ensure timely completion of documentation and support clinical innovation
Conduct move-in assessments and other assigned duties

Education

Board-certified Nurse Practitioner in at least two states: NV, AZ, CA
Valid DEA Certificate for Schedule 2-5 prescriptions
Completion of U.S. Medical Licensing Examination (physicians only)
