Nurse Practitioner - Northeast New Jersey - In Person Care
Quartet Health is looking for experienced Nurse Practitioners licensed in New Jersey to provide holistic care to individuals with serious mental illness. The NPs will be responsible for managing whole-person care and treatment for adult patients.
Skills
- Active nurse practitioner license and certification in New Jersey
- Expertise in diagnosis and treatment of patients with chronic and urgent medical/psychological conditions
- Excellent communication and computer skills
- Experience with Electronic Health Records (EHR)
Responsibilities
- Serve as the 'Quarterback' of the Care Team, providing longitudinal support and developing individual care plans
- Conduct initial intake visits, prescribe medications, and manage treatments
- Collaborate with the interdisciplinary team to ensure comprehensive patient care
- Address social needs and non-medical barriers to care
- Engage with diverse patients both in-person and via telehealth
Education
- Active nurse practitioner license and certification in New Jersey
- Bachelor's or Master's degree in Nursing with specialization in Family Nurse Practitioner or Psychiatric Mental Health Nurse Practitioner
