Family Nurse Practitioner - South New Jersey or Greater Philadelphia
Quartet Health is seeking Family Nurse Practitioners passionate about providing holistic care to those with serious mental illness. The NPs will be responsible for managing whole-person care and treatment for adult patients with serious mental illness, both virtually and in the home.
Skills
- Active nurse practitioner license and certification
- Expert in diagnosis and treatment of patients with chronic and urgent medical/psychological conditions
- Strong computer skills, including working with Electronic Health Records (EHR)
- Excellent written and verbal communication
- Consistent modeling of Quartet's values
- High levels of patient satisfaction
Responsibilities
- Provide longitudinal support of the patient care journey
- Develop individual care plans and assess patient progress
- Perform initial intake visits with patients
- Order, interpret, and analyze diagnostic tests
- Prescribe and manage medications and treatments
- Identify social needs and non-medical barriers to care
- Engage with interdisciplinary team for efficient care planning
- Develop strong rapport with a diverse set of patients
- Uphold clinical customer service through high-quality care
- Document all clinical findings and services provided
Education
- Nurse practitioner license and certification in good standing with the state of New Jersey Board of Nursing
