myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. In this role, you’ll be the bridge connecting prospective participants to the life-changing care they deserve. From conducting home visits and safety evaluations to partnering with our Outreach Specialists and IDT, your work will ensure every participant’s journey into PACE is seamless and empowering. What does success look like as our Enrollment RN?
  • Collaborative Teamwork: Partner with the Outreach Specialist and IDT to ensure a smooth and successful enrollment experience.
  • Thorough Evaluations: Conduct home visits to assess prospective participants for eligibility based on nursing home level of care requirements.
  • Safety Advocacy: Perform comprehensive home safety evaluations to ensure a safe living environment for prospective participants.
  • Informed Decision-Making: Participate in Enrollment Eligibility meetings to review and discuss Level of Care recommendations.
  • Expert Assessment: Complete Nursing Facility Level of Care (NFLOC) assessments and submit them to DHCS, making revisions as requested.
  • Meticulous Documentation: Maintain accurate and up-to-date records of all evaluations in CRM software.
  • Regulatory Excellence: Participate in audits by DHCS and CMS to ensure compliance and quality standards.
  • Retention Strategy: Collaborate with the Center Director to develop and implement participant retention initiatives.
  • Program Expertise: Stay informed and proficient in PACE Program eligibility, enrollment processes, and program specifications.
  • Community Engagement: Support business development by participating in outreach programs and promoting census growth.
  • Flexibility and Adaptability: Take on additional duties as assigned to meet program and participant needs.


What does an ideal candidate look like?
  • Compassionate Caregiver: A passion for delivering exceptional, coordinated care to frail elders and disabled older adults.
  • Educational Foundation: Associate’s degree from an accredited nursing school; BSN preferred.
  • Licensed Professional: Active Registered Nurse (RN) license in the state of California.
  • Life-Saving Readiness: Current CPR certification or the ability to obtain certification within 30 days of hire.
  • Experienced Advocate: 3–5 years of healthcare experience, with a strong preference for working with frail elderly populations.
  • Dependable Driver: A valid California driver’s license, reliable personal transportation, a good driving record, and auto insurance as required by state law.
  • Organized Communicator: Excellent organizational skills and exceptional written and verbal communication abilities.
  • Proficient Documenter: Strong documentation skills for CRM/EHR systems to ensure accurate records.
  • Bilingual Advantage: Bilingual fluency in English and Spanish is preferred to better serve our diverse community.
  • Certified for Care: Proof of current CPR/BLS certification or the ability to obtain it within 30 days of hire.
  • Compliance Ready: Proof of a valid California driver’s license, personal transportation, good driving record, and auto insurance as required by state law.

  • Location of the site is in Historic Pilipino Town (90057)
  • Schedule for the rule is Monday - Friday 8a-5pm and is hourly.  


$45 - $60 an hour

Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
