Description of Position:
- The Registered Nurse is responsible for the nursing services program, assisted by a licensed vocational nurse to meet the needs of each participant particularly in administering medications or monitoring of self-administration of medications by the participants; the use of measures to protect the participant from injury to self; and determining and providing the needs of the participant for personal care such as bathing, grooming, toileting and eating. A licensed nurse shall be on duty during the hours the center offers basic services.

Qualifications:
- The Registered Nurse shall be a person licensed as such by the California Board of Registered Nurse with a public health background.
- Able to speak/understand Chinese
Duties and Responsibilities:
- As a member of the multidisciplinary team, perform initial and continuing assessment of the participant and develop a written plan of care.
- Assess each participant to determine needs for personal care and assistance in activities of daily living such as grooming toileting and eating; include these needs in the plan of care and supervise the provision of these services. Specify nursing goals in each individualized plan of care.
- Document all skilled nursing care provided on the day the service is given and include quarterly signed and dated progress notes in each participant record.
- Administer medications or monitor self-administration of medications by participants.
- Provide training in self-administration of medications to participants.
- Monitor, administer and record prescribed medications based on established policy and procedures.
- Supervise the maintenance program by documenting services, training and staff time providing services.
- Provide liaison to the participant’s personal physician.
- Develop the nursing component in the plan of care for each participant capable of benefiting from a continence training program.
- Provide health education and counseling to the participant and family when indicated by the plan of care.
- Provide assistance to the participant in obtaining medical services from providers other than adult day health staff.
- Supervise personal care given by licensed vocational nurse and/or program aides.
- Maintain participant’s health record which includes:
- Signed quarterly progress notes from each member of the multidisciplinary team and outside consultants.
- Records of medications and treatments administered.
- Quarterly narrative nursing note, unless a participant’s condition requires a more frequent note, indicating participant’s progress toward achieving health goals.
- Make provision for emergency care, including arrangements for transportation.
- Notify the physician of any marked change in the participant’s conditions.
- Ensure that sufficient medical equipment and supplies are provided and properly maintained at all times, and available as needed, and as required under existing policy, rules and regulations.
- Destroy drugs in the center required to be destroyed witnessed either by another licensed nurse, administrator, pharmacist or physician.
- Uphold the purpose and philosophy which the ADHC operation is committed to achieve as reflected in the mission statement adopted by the corporation.
- Perform duties and responsibilities in accordance with company established policy, rules and regulations.
- Perform other duties as assigned by the Director of Nursing and/or Program Director.
