Discharge Planner

Sandstone Care is hiring a full-time Discharge Planner to support client engagement in their treatment care post-detox stabilization. Candidates must have experience in substance abuse treatment, be organized, detail-oriented, and able to function independently.

Skills

Previous experience in substance abuse treatment
Strong organizational and time management skills
Ability to work independently
Effective communication skills

Responsibilities

Lead discharge planning and summaries
Communicate with referents, treatment centers, and community providers
Discuss aftercare placement options with support from the Clinical Outreach & Aftercare Planning Manager

Education

Bachelors Degree in Psychology or related field strongly preferred
