Skills

Excellent communication skills
Proficient in Microsoft Office Suite
Experience with patient care coordination

Responsibilities

Perform outbound calls to patients for clinical needs
Coordinate resources and manage appointments
Follow up with patients to ensure care needs are met
Provide education materials to patients
Collaborate with clinical resources and care team

Education

Certification in BLS or CPR
Community Health Worker Certification preferred
