Care Coordinator

Strive Health is focused on transforming kidney care through a high-touch care model. We are looking for a Care Coordinator to join our team to support patients with chronic kidney disease.

Skills

BLS or CPR Certification
Verbal and written communication skills
Customer service experience
Intermediate MS Office proficiency

Responsibilities

Perform outbound calls to patients and providers.
Coordinate resources along the care delivery spectrum.
Follow up with patients to ensure their needs are met.
Provide education materials to patients.
Monitor patient hospitalizations and follow up as necessary.
