Skills

Excellent leadership skills
Strong clinical knowledge of CKD/ESRD
Effective communication abilities
Critical thinking skills
Experience with case management processes

Responsibilities

Supervise daily operations of intensive RN case management staff
Conduct employee performance reviews
Coordinate with staff to implement effective care plans
Oversee NCQA accreditation process through chart audits
Provide direct patient care and develop care plans based on patient needs

Education

Associate’s degree from an accredited school of nursing
