Strive Health aims to transform kidney care through a high-touch, coordinated care model. They are looking for a Licensed Practical Nurse (LPN) to support and educate chronic kidney disease patients.

LPN/LVN license
Patient care coordination

Responsibilities

Perform outbound calls to patients for clinical needs
Coordinate care delivery resources for patients
Monitor patient hospitalizations and follow up with care teams
Educate patients on their conditions and medications
Provide excellent customer service and support

Education

High school diploma or equivalent
Graduate of a state-approved nursing program
