Nurse Practitioner - Riverside, CA
Strive Health is looking for a Nurse Practitioner to join their interdisciplinary clinical care team, delivering advanced nursing care for CKD and ESKD patients. The role involves managing patient care in collaboration with a team of healthcare professionals to provide exceptional clinical care and patient experience.
Skills
- Master's or Doctorate level degree in Nursing
- National Board Certification
- Current state-specific licensure
- Minimum 2 years' experience as a Nurse Practitioner
- Experience in family/primary care, geriatrics, internal medicine, or related fields
Responsibilities
- Accountable for patient outcomes for a panel of CKD and ESKD patients
- Assess patient needs, deliver quality nursing care, and achieve optimal patient outcomes
- Collaborate with interdisciplinary team and external providers for patient care
- Follow best practices for chronic kidney care and evidence-based medicine
- Maintain current board certification and state-specific requirements
- Participate in on-call program
Education
- Master's or Doctorate level degree in Nursing
- National Board Certification
- Current state-specific licensure
- Current Drug Enforcement Administration (DEA) license
- Basic Life Support for Healthcare personnel (BLS) certification
