Responsibilities

Craft personalized care plans for patients with CKD and ESRD
Conduct regular assessments onsite and virtually
Manage a diverse panel of patients adhering to care management standards
Collaborate with care teams and deliver tailored education to patients
Utilize data for trend analysis and care optimization

Education

Associate’s degree from accredited school of nursing
Current RN license in the state of practice
