RN Care Manager (Hybrid)

Strive Health is dedicated to transforming kidney care through a high-touch care team. We are seeking an RN Care Manager to support individuals with kidney disease by delivering comprehensive coordinated care both remotely and onsite.

Responsibilities

Develop personalized care plans for patients
Conduct assessments and detect medical/social issues
Perform proactive outreach to patients
Manage a diverse patient panel and evaluate individual needs
Act as a liaison with dialysis facilities and update the care team
Provide culturally sensitive care and enhance communication among clinical teams

Education

Associate’s degree in nursing
Current RN license in the state of practice
