Devoted Medical Group was founded on the belief that if we treat each member like we would our loved ones, we can meaningfully improve healthcare experiences and outcomes for some of America’s most vulnerable patient populations. The Intensive Home Care (IHC) Program is at the heart of this goal: we are a highly collaborative, multidisciplinary team of physicians, nurse practitioners, case managers, social workers, and community health workers united by our mission to build a better care delivery system for Devoted’s most complex members. Through a mixture of in-home and virtual interventions, we provide whole-person clinical, social and logistical support for patients with complex chronic conditions, frail elders, and patients at high risk of hospital admissions.
The Community Health Worker Supervisor (CHW Supervisor) is vital to the success of the Intensive Home Care Team, charged with building, managing, and inspiring a high-performing team of Community Health Workers, who are accountable for building trusting patient relationships and empowering patients to live healthy, dignified, independent lives. Additionally, our CHW Supervisor, serves alongside managers from our other disciplines (NP, RN, SW, OC) to form a regional interdisciplinary management team, responsible for the day-to-day management and clinical, operational, and cultural successes of 10 pods serving approximately 900 high risk patients. This role will be responsible for managing the IHC CHW (front-line patient advocate, community resource connector, care navigator & coordinator, and wellness and life skills coach), provide support to team in breaking down barriers for patients, develop new workflows and job aids, facilitate coaching and provide recommendations for improving performance, track and report operational processes and productivity, prepare reports and documents for leadership, and attend and/or facilitates regular meetings with team members and leadership as required.
Responsibilities will include:
Tracking staff performance through competency check-offs, audits, shadowing virtually/in person, and 1:1 meetings; conducting coaching and providing support as needed
Identifying needs, problem solving, and solutioning with the leadership team to find improvements and standardizing the clinical workflow
Creating workflows, assessments, and plans of care that create efficiencies for the team and show outcomes for patients
Assisting the team in working with escalated patient needs
Coordinating and facilitating ongoing education to existing staff and an on-boarding experience for new teammates
Travel to team’s geographies to shadow home visits
Partnering with multiple cross-functional teams to drive execution and streamline workflows across programs
Meeting with the IHC Leadership regularly to discuss team performance, successes, and areas for improvement

You are confident in training teams on how to manage patient needs so that they do not fall through the cracks
You have a background in providing clinical, psychosocial, and/or logistical support to the underserved or elderly population
Comfort with interpreting and acting on data
You’re a great team member with a can-do attitude
A learning mindset; comfortable operating in a fast moving environment
Proven leadership, agility, and collaboration skills
Must haves:
Experience managing teams and comfort with managerial tasks of precepting, coaching, and providing constructive feedback
Community Health Worker Certification, Certification of Medical Assistant (CMA), Registered Medical Assistant (RMA), or an Associates degree or higher in health sciences or related field and 5+ years’ work experience in a healthcare setting
Two (2) years of experience in outpatient medical care, with a bonus if you have experience with telehealth or house call visits. The ability to comfortably multi-task: you’ll be listening, talking and typing all at the same time.

Salary Range: $60 - 68K annually
