Wound Care Nurse Practitioner

Theoria Medical is a comprehensive medical group seeking dedicated nurse practitioners to provide high-quality wound care services at skilled nursing facilities.

Skills

Valid Nurse Practitioner license in the state of practice
NP degree from an accredited institution
Active DEA license
Post-Acute/SNF knowledge or experience preferred
Familiarity with EMR systems and charting practices

Responsibilities

Manage wound care of the population at the assigned SNF
Collaborate and round with wound care nursing staff
Provide high-quality, patient-centered, comprehensive wound care services
Timely documentation of patient care/encounters
Available for consultations with facility nursing staff
Complete provider documentation in proprietary EHR system

Education

NP degree from an accredited institution
