Summary

Works under minimal supervision to coordinate and maintain scheduling of visits for home health patients and staff. Enters staff and patient information into database and maintains reports.

Experience and Education

High school graduate or equivalent level of education required.
Home Health/Scheduling experience: 6 months required.
Computer skills: 1 year required.
Candidates must be authorized to work in the United States.

Job Duties

Assists in taking referrals, patient intake process.
Is responsible for development and continual monitoring of the work schedules for Clinicians to meet patient care needs, adjusts staffing according to available staff and reports changes to appropriate supervisors, assure accuracy of the time reports, verify hours worked daily, bonus hours, overtime hours and benefit days, and calculate staffing based on adjusted census daily.
Is responsible to coordinate and maintain scheduling for patients and staff.
Timely communication of scheduling changes between office and field staff.
Creates and maintains staff schedules. Prepares weekly schedule for field staff and provides notification to employees.
Provides staffing for sick leave, vacation, long term leave. Schedules patient appointments/visits according to care plans and staff availability.
Discerns patient services required as outlined in agreements, urgent requests, and care plans.
Contacts care providers and clients regarding day-to-day changes.
Enters staff and patient information into database.
Maintains staff and patient statistics, and reports.
Calls hospital Social Services department to notify them that an Agency patient has been admitted to their hospital.
Performs general office duties including but not limited to word processing, photocopies, etc.
Performs other duties as assigned.

Working Conditions

Work is performed primarily in the home health office. The candidate will need to be available to be on-call two weeks/weekends per month to manage any shift changes and nurse call-offs.

Any qualifications to be considered as equivalents in lieu of stated minimums require prior approval of the Vice President for Human Resources Administration, or his/her designee.
