Skills
Strong technical skills in Microsoft Suite applications and EMR systems
Excellent leadership and organizational skills
Effective communication with seniors, families, and healthcare providers
Responsibilities
Provide high-quality customer service to enrolled PACE participants
Act as a liaison between providers and participants
Assist with grievance process and maintain open communication
Attend and lead interdisciplinary care team meetings
Maintain tracker to identify immediate needs
Education
High school diploma or equivalency
