WelbeHealth is dedicated to providing seniors comprehensive medical services through a collaborative interdisciplinary team approach. They are looking for a Primary Care Physician who will perform comprehensive assessments and coordinate participant care.

Responsibilities

Provide assessment, planning, primary care, and continuous medical coverage to participants
Collaborate with community-based physicians and IDT in discharge planning
Participate actively in IDT to communicate participant changes
Maintain care of participants in nursing homes
Participate in rotating night and weekend call

Education

Doctorate Degree
Successful completion of an accredited residency program
