About the role
As a Care Coordinator working in the IHH (Integrated Health Home) program, I am responsible for considering the clients' whole health when helping them select services and providers that are right for them. I coordinate meetings with clients and their care teams in order to develop person-centered treatment plans and use these plans to request the proper funding through MCOs and through the clients' counties. These plans are based off of assessments. I address the needs of clients on an individual basis and advocate for clients when meeting with providers, health professionals, educational professionals, and legal professionals. In this job, it is important to establish positive, professional relationships with clients and to maintain strong communication between clients, care teams, and the IHH team. It is also important to maintain proper documentation and be aware of many deadlines at the same time.

Overnight Youth Associate

About the role
As an overnight youth associate, I was responsible for coordinating and running the nighttime and morning routines in the residential communities. I built rapport and established positive relationships with children aged 5-17 while maintaining professional boundaries. While working in a community, I monitored and recorded the progress and barriers of clients, assisted clients with their daily living routines, mitigated escalations, verbally de-escalated clients, and provided opportunities for meaningful activities. This job also required staff to work together and communicate effectively in order to maintain an environment that would promote the safety and overall success of our clients. Working in a residential setting with juveniles aged 5-17 who had mental and behavioral disorders for the past year and a half has strengthened my ability to build rapport, build strong relationships, and set professional boundaries with children in this age range. It has also strengthened my ability to problem solve under pressure and to understand how to approach potentially volatile situations involving youth. I also learned how to properly work with clients who have suffered through trauma. The knowledge and experience from this job has translated into my current job working with child, adolescents, and adults.

About the role
As a customer service associate, I mainly worked at a cash register. Aside from handling transactions, I was also responsible providing guests with a positive experience. This meant maintaining a positive attitude and utilizing patience, even when faced with difficult situations. This job laid the foundation for developing the problem-solving skills that I have used throughout my career.
