As a Registered Nurse (RN) Patient Care Coordinator at Avera @ Home Referral Center in Sioux Falls, SD, you will play a crucial role in shaping the patient experience from hospital to home. This fun and fulfilling position allows you to work regular day shifts from 8 AM to 4:30 PM, with every fifth weekend off, making it a perfect balance between work and personal life. In this dynamic role, you’ll be part of a compassionate, multidisciplinary team dedicated to moving health forward for our patients. Your responsibilities include acting as a home care expert, facilitating seamless transitions from hospital to home healthcare or hospice services. You will provide crucial clinical assessments and support referral sources in identifying the right services for patients in a post-acute setting. Your keen understanding of both skilled and non-skilled home care services, alongside excellent communication skills, will enable you to coordinate effective care plans in collaboration with physicians. Additionally, you'll document patient care plans and identify areas for improving continuity of care. This role requires an active RN license in the state of practice and preferably 1-3 years of home health or hospice experience.
