As a Care Advocate on our team, you will be a trusted voice and support system for our patients, helping them navigate their care–within and outside of Color–to stay ahead of cancer. You’ll make a direct impact by educating and motivating patients to take action to prevent and reduce their risk of developing cancer, as well as make sure they get screened and catch cancer early. You will make it as simple as possible for them to access care, meeting them where they are in their healthcare journey. You will be responsible for making sure patients get the answers, resources, and care they need when it comes to cancer screening and prevention. If you’re passionate about delivering compassionate, high-quality support in a fast-moving healthcare environment, this role is for you. 



How You'll Contribute:
  • Connect with patients to learn about their unique health history and current needs as it relates to cancer prevention, risk reduction, and screening
  • Provide basic education and counseling on cancer risk reduction and prevention
  • Develop cancer screening and risk reduction plans for patients to be reviewed and approved by Color Medical’s clinicians
  • Triage patients as necessary to other care team members at Color (e.g. genetic counselors, clinicians, dieticians), based on their needs 
  • Navigate patients to the right external care setting, if not available at Color
  • Motivate patients to complete medical and non-medical action plans, while encouraging general wellness 
  • Build long-term, trust-based relationships with patients to continuously engage them on cancer screening and risk reduction efforts
  • Schedule patients for external appointments and coordinate their care seamlessly
  • Deliver gold-standard service to patients via messaging, phone, and video 
  • Other responsibilities as needed


Our Ideal Candidate Will Have:
  • A background in allied health, human services or healthcare, with 3-5 years of experience supporting patients.
  • Background and extensive experience counseling and working directly with patients
  • A friendly, empathetic problem solver who is passionate about helping people live a healthier life, and who are eager to help people stay ahead of cancer through prevention, risk reduction, and early detection
  • Team-focused; effective collaborator within the care team.
  • Exceptional written and verbal communication, with a focus on empathy and active listening
  • Strong clinical skills, and high bar for experience and quality of care
  • Excellent organizational skills with the ability to multitask and thrive in fast-paced environments
  • Comfort with telehealth technology, including video chatting, electronic health records, patient messaging, and other digital tools


Nice to Have:
  • Clinical experience in roles such as nursing, cancer counseling, health coaching, social work, working as a medical assistant or related fields
  • Experience supporting patients in the cancer space
  • Experience in care coordination 
  • Experience working in a fast-paced startup environment
  • Experience with motivational interviewing in healthcare

Base Salary Range: The actual base pay is dependent upon many factors, such as: work experience, market data, skills, geographic location, and business need. The base pay range is subject to change and may be modified in the future. This role may also be eligible for bonus, equity, and benefits.
