The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
The primary role of the Residential Services Case Manager is to provide care to disabled individuals in the designated single “program units” of the CCH housing facilities and/or in off-site units. The ultimate goal is to assist these individuals in maintaining their housing and to provide the services, linkages, and interventions necessary to prevent them from returning to homelessness and support recovery from homelessness, addiction, and behavioral health concerns. This position will be responsible for the orientation, assessment, goal planning, and service provision to adult individuals with disabilities as well as programming and activities for the client community.
