Program Coordinator I

Description

Areas of Responsibility:

  • Complies with all company and department policies and procedures; state, federal, local and other third-party payer policies; and applicable standards of practice.
  • Demonstrates customer service.
  • Performs competency evaluations of all staff members.
  • Provide education to staff on performance improvement.
  • Plans and organizes training for department staff members.
  • Documentation must meet current standards and policies.
  • Perform management activities including interviewing, hiring, evaluation, and department staffing and scheduling.
  • Maintains all state and federal regulatory requirements related to Innovate and to the facility.
  • Maintain patient confidentiality at all times.
  • Attend committee, CQI and management meetings, as appropriate.
  • Train personnel utilizing on the job training for nursing and other facility staff.
  • Ensure compliance with policies and procedures regarding department operations, fire, safety and infection control.
  • Maintain standards of professional society procedures and ethical behavior and comply with all organizational policies.
  • Resolve personnel concerns at the department level, utilizing the grievance process as required.
  • Effectively and consistently communicate administrative directives to personnel and encourage interactive departmental meetings and discussions.
  • Represent the organization in a positive and professional manner.
  • Responsible for direction of the Rehabilitation Services Department staff.
  • Able to observe and evaluate treatment effect, recommend change to physician if needed.
  • Able to perform appropriate assessments on all patients as related to the therapy requested and provided and reassessments as per policy.
  • Formulate a teaching plan based upon identified learning needs.
  • Demonstrate knowledge and proficiency of all rehab services modalities.
  • Function as liaison between administration, patients, physicians and other healthcare providers.
  • Manage and operate equipment safely and correctly. Maintain department cleanliness and safety. 
  • Comply with ongoing departmental inspections of equipment and physical space.
  • Maintain records pertinent to personnel and operation of the department.
  • Communicate appropriately and clearly to physicians, staff and administrative team.
  • Coordinate and direct patient care to ensure patients’ needs are met and facility policy is followed.
  • Prepare reports regarding rehab services, i.e., utilization, performance improvement, service data, outcome data, productivity, including long term capital budget and short-term operational budget.
  • Demonstrate an ability to be flexible, organized and function under stressful situations.
  • Maintain a good working relationship within the department and with other departments.
  • Plan and manage treatments and documentation of patients with varying payers in electronic medical record.
  • Float to other facilities as needed. All other duties as assigned.
  • Complies with areas of responsibility associated with employee's specific professional discipline (PT, OT, SLP, PTA, or COTA)
