About the role

As a Social Worker, you’ll provide vital support to members through three 55-minute direct sessions each day, offering counseling, guidance, and advocacy to help them navigate services, develop treatment plans, and access essential resources. You'll also work closely with families, communities, and employers to foster meaningful support networks.

You will  support the Senior Social Worker with child protection reviews, and onsite mental health and social escalation support. You’ll collaborate with the Mental Health & Wellbeing, SSC, and Nursing teams to ensure professional and ethical standards are met while maintaining compliance with regulations and treatment plans.

Finally, you will also play an important  role in our SSC, handling mental health and social work escalations. This will include scheduled weekly shifts during core business hours, Monday to Friday.

What you’ll be doing

  • Deliver social work services within Sonder’s stepped care model, offering guidance and intervention at the second level of support.
  • Perform psychosocial assessments to identify member needs, barriers to care, and appropriate support pathways.
  • Create tailored plans with service referrals and actionable steps to help members access necessary support.
  • Proactively link individuals to relevant community services, government agencies, and charitable organizations.
  • Work closely with internal and external providers to implement effective management plans for members.
  • Serve as a key escalation point for mental health and social issues, offering immediate support and intervention for complex cases.
  • Maintain professional integrity by adhering to regulations, ethical guidelines, and treatment plans.
  • Participate in clinical supervision, training, case reviews, and quality assurance initiatives to enhance service delivery.

What you bring to the team;

  • A minimum Bachelor's degree from Australia or New Zealand in Social Worker or equivalent is essential 
  • Valid registration of Social Work in Australia (AASW) and New Zealand (NZSWRB) 
  • Relevant experience in Social Work and/or similar case management roles.
  • Prior experience working across a variety of mental health issues, age groups and cultural backgrounds.
  • Ability to complete psychosocial assessments and to formulate and implement appropriate management plans
