As a Home Health LVN-PRN, your main responsibilities include performing skilled visits, developing and revising care plans alongside the RN Case Manager, and providing quality patient care based on individual needs. You’ll also observe your clients' conditions, document changes, and provide guidance to families and health team members throughout the treatment process.

To qualify for the Home Health LVN-PRN position, candidates must possess a valid Texas Nursing License and have experience in geriatric home health care. Additionally, strong organizational and customer service skills are essential, along with a valid driver’s license to travel between patient homes.
