Home Health LVN-PRN

Seeking Home Health Full-time -LVN in Texarkana, TX and surrounding areas! (Daingerfield, Lone Star, Avinger, Linden, and Ore City.)

Responsibilities

  • Skilled Visits
  • Assists in development, review and revision of the plan of care
  • Provides quality patient care set forth by the RN Case Manager's Care plan
  • Observes, records and reports reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician.
  • Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment

Requirements:

  • Geriatric Home Health experience
  • Texas Nursing License
  • Strong organizational and self-management skills
  • Strong and compassionate customer service skills
  • Valid driver’s license
