Aftercare Recovery Support Specialist

SUMMARY

Under the supervision of the Vice President of Patient Access, conduct outreach to individuals who leave residential treatment and provide support/resources as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Conduct outreach calls via phone to individuals who have left OASAS residential treatment.
  • Collect information regarding their status in recovery and their well-being.
  • Document outreach attempts in EMR
  • Provide support and resources when applicable.

REQUIRED EDUCATION

  • High School Diploma or Equivalent Required

LICENSES/CERTIFICATIONS

  • Certified Recovery Peer Advocate (CRPA) or Credentialed Alcoholism and Substance Abuse Counselor Trainee (CASAC-T) in good standing.

REQUIRED EXPERIENCE

  • Minimum one year experience providing peer support services

WORK ENVIRONMENT

  • This job is a remote position. This role routinely uses standard office equipment such as computers, phones, and tablets.
