Job Title: Social Worker

About the Role:
We are currently seeking a highly motivated Social Worker. This role will report to a Manager - Care Management and enable us to continue to scale in the healthcare industry.
What You'll Do:
- Assesses the psychosocial status of members/patients and families/caregivers related to the patient/member’s mental conditions and environment and communicates findings to the other patients/members of the interdisciplinary group.
- Provides an assessment of patient/member’s psychosocial and living status and assistance when it is not safe for the patient/member and another plan is required.
- Customarily and regularly exercised discretion and independent judgment in the performance of duties
- Carries out social evaluations and plans intervention based on evaluation findings. Counsel patient and family/caregivers as needed in relationship to stress, anxiety, and other identified coping difficulties.
- Be open to over time for after hours and on the weekend via email and phone to assist in crisis intervention and reporting, e.g. abuse, unsafe living environment, and suicidal ideation.
- Maintains clinical records on all patients referred to social work.
- Provides information and referral services for organization patients and families/caregivers regarding practical and environmental/social determinants of health needs.
- Provides information to patients and families/caregivers and community agencies.
- Serves as liaison between patients and families/caregivers and community agencies.
- Maintains collaborative relationships with organization personnel to support patient care.
- Maintains and develops contracts with public and private agencies as resources for patient and personnel.
- Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings.
- Educate physician and other team members in understanding significant social and emotional factors related to health problems and death/dying/end of life issues
- Actively participates in quality assessment performance improvement teams and activities.
- Manages the assessment and delivery of counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
- Plans for Behavior Health Program development, expansion and refinement through annual evaluation of services.
- Provides social information and referral services to members/patients.
- Manages community resource list as needed.
- Evaluate and plan services to meet case management requirement, prepare care plans for multiple insurance companies.
- Functions independently, but seeks and accepts guidance from other members of the interdisciplinary group, or from member of the community.
- Prepare clinical updates for monthly provider meetings.
- Other duties as delegated by the Ambulatory Care Department Manager
Qualifications:
- A Master’s degree in Social Work. Meets personnel qualifications stated in §418.114(b)(3)
- 1+ year of managed care experience
- Demonstrates an ability to supervise and direct professional and administrative personnel
- Ability to deal tactfully with customers and the community
- Knowledge of corporate business management, governmental regulations and Joint Commission standards
- Demonstrates good communications and public relations skills
- Demonstrates autonomy, organization, assertiveness, and flexibility and cooperation in performing job responsibilities
- Must have clinical experience relevant to the psychosocial needs of patients and families

Environmental Job Requirements and Working Conditions:
- This position is remotely based in Maryland.
- This position would require traveling once in a while to certain facilities.
- The total compensation target pay range for this role is: $37.00 - $41.00 per hour. The salary range represents our national target range for this role.
