Starting Compensation: $25.13/hr + DOE + DOQ
Schedule: 8:00am - 4:30pm Monday - Friday
Preferred Qualifications:- Bachelor's Degree in human services, social work, sociology preferred. Can substitute years of experience for degree requirements.
- Three years of case management or social services experience; and/or equivalent combination of education and experience preferred.
- Knowledge of and experience in using trauma-informed care principles required.
- Demonstrated experience in mediation between persons exhibiting escalated behavior required.
- Experience building and maintaining relationships with community partners.
- Must have valid Oregon Drivers license; must have been a licensed driver for at least 2 years; must have a driving record that permits coverage under C@P’s company insurance.
Essential Job Duties Include (But Not Limited To):- Oversee and facilitate the C@P waitlist, using diversion as a strategy to help people get to the shelter that best meets their needs.
- Coordinate intake for Safe Parking applicants.
- Build and maintain relationships with churches that function as Safe Parking sites; address issues that arise and triage crisis situations in partnership with the C@P Safety/Security Team.
- Complete Coordinated Entry assessments with Safe Parkers, in collaboration with Salem Housing Authority, The ARCHES Project, and other service providers to help participants receive housing assessments and assist guests in updating Coordinated Entry information for placement in emergency or permanent housing.
- Make referrals for clients regarding drug and alcohol treatment, domestic violence services, health services, and other services.
- Assist clients in obtaining a birth certificate, ID, free phone, transportation vouchers, Oregon Health Plan, and other resources to prepare them for housing.
- Communicate and work closely with the Shelter Managers and Assistant Managers to establish continuity of care.
- Create Action Plans with participants to overcome challenging behaviors and address any lack of engagement in services.
- Attend required training and team meetings, and learn policies and procedures for C@P operations.
- Track what services are needed by each participant and help participants to establish goals and objectives.
- Help participants connect with workforce development workshops and resources.
- Enter data in HMIS and assure data quality. Run data reports when applicable.
$25.13 - $25.13 biweekly
Work environment:
Indoor/Outdoor work environment with frequent interruptions and demands, and occasional unpredictable behavior. Must be able to push, pull, and/or lift 15 pounds or less. Driving between Church at the Park operated and managed sites.
Work environment:
Close quarters with vulnerable people including individuals without stable living situations who may be struggling with mental health and substance use concerns.
