The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
The Colorado Coalition for the Homeless seeks a Bilingual (Spanish) Case Manager to work collaboratively as a member of the integrated health care teams situated within Stout Street Health Center (SSHC) to provide patient-centered case management services. These services include providing help with housing, public benefits, medical/mental health/substance treatment referrals, obtaining identifying documents, payee service referrals and other needs with the goal of ending their homelessness. This position also develops working relationships within and outside of the organization to improve client access to needed services, and maintains documentation of all services provided. This position is located at 2130 Stout St. Denver, CO 80205 and reports to the Program Manager.
