The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
The Colorado Coalition for the Homeless seeks a Case Manager I to provide comprehensive case management services to American Indian and Alaska Native (AI/AN) individuals, households, and families who are experiencing homelessness in Metro Denver, with the overall goal of decreasing health disparities and increasing health and wellbeing among this population. Principal focus is linking participants of CCH Talking Circle ceremonies and other Native American Services clients with integrated primary and behavioral health care services provided at CCH's Stout Street Health Center (SSHC). To address the spectrum of social determinants of health, additional services provided include but are not limited to: crises intervention, provision of basic needs (socks, food, cold weather gear etc.), facilitation of increased access to emergency shelter, transportation, client advocacy, and linkage to legal services, public benefits, identification, substance treatment, and housing. This position is located at 2100 Broadway, Denver, Colorado 80205 and reports to the DOES Program Manager.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Licensed health care providers may earn funds towards student loans in exchange for a two-year commitment at an NHSC-approved site through this program.
[Language for RN/LPN postings only – Remove if not needed] Qualified candidates are eligible for the NURSE Corps Loan Repayment Program which offers payment of 60% of unpaid nursing education debt in exchange for a two-year commitment at a NURSE Corps approved site with an option to extend to a third year for an additional 25% of the original balance.
