Care Coordinator (Remote IDT)
Evergreen Nephrology is seeking a compassionate Care Coordinator to support kidney care through a holistic and person-centered approach, aiming for high-quality patient outcomes.
Skills

Responsibilities
- Establish trusting relationships with patients and families
- Coordinate care with interdisciplinary team members
- Monitor treatment plans and educate patients
- Link patients to community resources
- Participate in establishing care plans
Education
- Clinical experience such as EMT, MA, CNA, or Phlebotomy certification is a plus
