FORM Health is a virtual obesity medicine clinic seeking an Advanced Practice Professional experienced in obesity medicine to provide longitudinal Telehealth care for patients.
Skills
- Nurse Practitioner or Physician Assistant degree
- Experience practicing Obesity Medicine
- Licensed to practice and prescribe in Florida and one of MA, NY, TX, or IL
- At least 2 years of Obesity Medicine experience
- Excellent interpersonal skills
- Commitment to evidence-based approach to obesity management
- Prior experience with telemedicine visits
Responsibilities
- Provide longitudinal Telehealth care for patients with obesity
- Collaborate with physicians and registered dietitians towards exceptional team-based care
Education
