Home Care Assessment Nurse (RN)
Givens Home First is seeking a dedicated Home Care Assessment Nurse (RN) to join their growing team. You will play a vital role in enhancing client care through assessments and care coordination.
Skills
- Current RN license in North Carolina
- Experience in home care or home health with a focus on geriatric care
- Proficiency in WellSky or similar software
- Strong work ethic and punctuality
- Ability to work flexible hours
Responsibilities
- Complete initial client assessments and supervisory visits.
- Maintain and adjust individualized care plans.
- Work closely with the care team to provide input and feedback.
- Act as a liaison with clients and families.
- Document assessments and care plans using WellSky software.
- Conduct supervisory visits to evaluate caregiver performance.
- Provide on-call support for caregivers and clients.
Education
- RN degree from an accredited institution
