Onehome is looking for Health Health Aides/CNA .

Job Summary: The Home Health Aide/CNA provides assistance with daily living and personal care services in the patient's home in accordance with an established care plan and under the supervision of a Registered Nurse of other designated health care professional.

What to expect in your role:

• Assisting with personal care, hygiene, and activities of daily living
• Encouraging patient participation in activities to the extent to which the patient is able
• Assisting with ambulation as instructed/needed
• Reporting any changes in the patient's condition, living conditions etc. to RN/Supervisior as they occur
• Complying with all policies and procedures including, but not limited to HIPPA Privacy Rule
• Following care plan as instructed
• Maintaining patient care records and documenting provided services
• Accepts only those assignments for which they are qualified.
  

What you need to succeed:

• High school degree or equivalent
• Evidence of formal training and/or certification as Home Health Aide/CNA as per state law
• CPR Certification with American Heart Association or American Red Cross
