Care Navigator - Douglas, Minnesota (Hybrid)

Homeward is a technology-enabled healthcare provider delivering quality, affordable and comprehensive care to rural communities. They are looking for a full-time Care Navigator (MA or LPN) to help individuals in Douglas, Pope, and Grant counties achieve their best health.

Skills

Completion of a Medical Assistant program
High school diploma or equivalent
At least two years of experience in patient-facing roles
Passion for delivering care in rural America
Experience working with seniors and persuading them on health actions

Responsibilities

Support fulfillment of recommended health services
Mitigate administrative and logistical barriers to obtaining health services
Engage potential members and build member health literacy
Coordinate patient flow and enter information into medical records
Collaborate with primary care providers and community resources

Education

Completion of a Medical Assistant program
High school diploma or equivalent
